Overview

In recent years, there has been increasing recognition of the need to address social determinants of health (SDOH) — such as housing, food, social support network and transportation — through a more comprehensive and equitable health care system. Addressing SDOH through the health care framework not only leads to better health and quality of life for patients, but also lower health care costs for plans and providers.

Join us for the webinar where we will provide insights into local plans’ SDOH initiatives. Based on targeted research and interviews, representatives from the JSI Research & Training Institute, Inc. will provide a summary of all plans’ SDOH investments. Participants will also hear from local plan leaders who will detail how they developed their respective SDOH investments, funding strategies, and tools for measuring impacts and return on investment. Don’t miss this opportunity to learn about how plans are addressing SDOH, the impact of their work on patients and the plans, and how to continue on the path toward continued investment in SDOH.

Cancellation Policy: Cancellation notification must be submitted in writing to institute@lhpc.org seven or more days prior to the course date. Refunds will not be given after that date. Substitutions are encouraged.
