Overview
The LHPC Institute and California Health Care Safety Net Institute (SNI) are pleased to invite you to a convening of Medi-Cal managed care plan and public health care system leaders.
The transition from the hospital to a new care setting is a tenuous time when patients are at increased risk of complications and readmission. Both managed care plans and public health care systems play an important role in safely discharging patients from the hospital and ensuring they receive appropriate placement and follow-up care.
Join us for this important convening where plan and system leaders will:
- Examine plan and system roles and responsibilities in the discharge process and identify effective staffing models for coordinating transitions across care settings
- Learn how plans and systems implement clinical workflows and data exchange processes to support safe transitions home or to lower levels of care (e.g., skilled nursing facilities, recuperative care, assisted living, etc.)
- Identify promising strategies to discharge patients with complex health and social needs, such as homelessness, severe mental illness and dementia
- Understand how plans and systems are partnering to meet new requirements under CalAIM’s Population Health Management Transitional Care Services
- Strengthen relationships with plan and system partners through joint planning efforts
Who Should Attend:
Ideally, participants will have a decision-making role in efforts related to hospital discharge, placement in community settings and follow-up care. This may include chief medical officers, clinical, population health management and informatics leaders and others who oversee or support case management or hospital relations activities.

NOTE: Each invited plan and public health care system may elect to register up to four (4) participants per entity. Plans and public health care systems are strongly encouraged to coordinate attendance prior to registering.
There is no cost to attend the event; however, registration is required.
Registration Deadline: Registration closes Oct. 29 at 5:00 p.m. or once capacity is reached.
Registration Confirmation: Registrants will receive an email notification confirming their registration.
Program Materials: Presentation and other program materials will be made available electronically only and will be provided to confirmed registrants.
