Overview
Patients who are attentive to and engaged in their care experience better health outcomes, often resulting in positive impacts on quality performance for health plans and public health care systems. Getting and keeping patients engaged can be challenging, prompting the need for more collaborative and coordinated outreach efforts.
Join the LHPC Institute and California Health Care Safety Net Institute (SNI) for an upcoming webinar where plan and system partners will describe their respective strategies for reaching unengaged patient populations and how these joint efforts led to notable improvements.
Who Should Attend:
Chief Medical Officers, Chief Health Equity Officers, quality leaders and staff, member services and others involved with or engaged in patient outreach and engagement activities and initiatives.
Agenda
During the webinar, participants will:
- Understand how to identify and tailor outreach for Assigned Not Yet Seen (ANYS) and unengaged patient populations
- Learn practical outreach strategies that address barriers to care
- Identify joint approaches for local health plans and public health care systems working together to improve patient outreach and engagement
- Hear real-world examples of collaborative efforts:
- Gold Coast Health Plan and Ventura County Health Care Services will share how embedded health plan staff work with the systems’ ambulatory care quality team to close care gaps.
- Inland Empire Health Plan and Riverside University Health System will discuss how they leverage direct EHR access to schedule ANYS patients and embed Community Health Workers at schools to engage families in care.
